Symptoms and context
Rosacea is a chronic inflammatory condition of the central face. Four subtypes:
- Erythematotelangiectatic — persistent redness, visible vessels.
- Papulopustular — red bumps and pustules that look acne-like.
- Phymatous — thickening of the skin (e.g., rhinophyma).
- Ocular — burning, gritty eyes; commonly under-diagnosed.
Common causes & triggers
- Genetics and vascular reactivity.
- Demodex mite over-colonization (drives the papulopustular form).
- Triggers: heat, sun, alcohol (especially red wine), spicy food, stress, hot drinks, exercise, abrupt temperature changes.
- Aggravators: harsh skincare, fragrance, alcohol denat, strong actives.
Routine principles
- Identify and reduce triggers before optimizing skincare.
- Bland is friend. Skip fragrance, essential oils, alcohol denat, menthol/camphor.
- Use anti-inflammatory actives — azelaic acid, niacinamide, centella.
- SPF every day. UV is a top trigger.
Morning routine
- Lukewarm water rinse or cream cleanser.
- Niacinamide 4–5% serum.
- Ceramide moisturizer.
- Mineral SPF 30–50 (zinc-based; sometimes green-tinted to neutralize redness).
Evening routine
- Cream cleanser.
- Azelaic acid 10–15% or prescription ivermectin/metronidazole.
- Ceramide moisturizer.
Ingredients to prioritize
- Azelaic acid 10–20%.
- Niacinamide.
- Centella asiatica, madecassoside.
- Hypochlorous acid sprays (for active flares).
- Ceramides, panthenol.
- Prescription topicals: ivermectin (Soolantra), metronidazole, brimonidine (transient redness reducer), oxymetazoline (Rhofade).
Ingredients to use carefully
- L-ascorbic acid at high strength — can sting on flaring skin; try ester forms.
- Retinoids — useful in some cases, but introduce only after rosacea is controlled.
- Strong AHAs/BHAs.
- Fragrance, essential oils, witch hazel, menthol.
- Physical scrubs.

When to see a professional
- Persistent flares despite trigger management.
- Visible vessels (telangiectasia) — vascular laser/IPL can help.
- Eye involvement (ocular rosacea).
- Skin thickening (phymatous changes).
Dermatologists offer prescription topicals, oral therapies (low-dose doxycycline, isotretinoin in select cases), and energy-based treatments. A targeted plan beats long-term OTC trial and error.
